AFFIRMA looks at each rehabilitation residents’ functional ability as a whole and measures rehab gains in all 3 therapy disciplines – physical, occupational and speech.
We provide comprehensive outcome reports, which demonstrate the success of our clinical programs and the accomplishments of residents. These reports can be customized at both the facility and individual level, and are shared with the facility and interdisciplinary team members. Additional details such as physician group and payer source can also be included in reports.

Our goal is to provide a seamless continuum of care by working with residents and families to ensure a smooth transition to home, residential care, or a long-term care setting. We pay special attention to achieving the highest outcome via personalized treatment plans with one of the highest return home rates in the industry.
